New Patient Paperwork
If you decide to make St. Augustine Family Dentistry your new dental home, we'll assist you in scheduling your first official appointment with our dentist. Prior to this appointment, we ask all new patients to arrive at least 20 minutes early to fill out some paperwork. These forms request basic contact information, as well as more detailed information regarding your dental health history. If you prefer, you can download the forms at the following link and complete them on your own time, eliminating the need to arrive early for your appointment.
Our dentist will thoroughly review your new patient forms, paying close attention to the details of your dental health history, before commencing with your exam. The physical exam allows our dentist to assess your oral health and look for symptoms that may indicate tooth decay, gum disease or a more serious dental health issue. After the exam, our hygienist will perform a thorough teeth cleaning to remove any plaque and tartar buildup from your teeth and gums.
At your new patient exam, our dentist will take digital x-rays of your mouth. This will allow our dentist to closely examine your tooth roots and bone structure to look for signs of cavities, infection, bone deterioration and more. The x-rays take just a few minutes to complete and the images are immediately available for our dentist to review.
When your exam, cleaning and x-rays are complete, our dentist will discuss any findings with you and recommend additional treatments you may need. In some cases, you may have multiple options to choose from, so our dentist will review all potential options with you and recommend the one that will provide you with the most benefit.
If additional treatments were recommended, our staff will schedule your follow-up appointments in accordance with your availability before you leave our office. If no additional treatments are required, we'll schedule your next exam and cleaning six months from now.
